Description

Amaryl M Forte is an antidiabetic medication prescribed to patients with type 2 diabetes mellitus who cannot improve glycemic control with a strict diet alone or in combination with a therapy based on one antihyperglycemic agent.

It contains 2 active ingredients, Glimepiride and Metformin, which provide a glucose-lowering effect in different ways due to the difference in their modes of action. Glimepiride is an insulin secretagogue that prevents blood glucose spikes by enhancing the release of insulin from the pancreas and improving the response of the body to insulin. Metformin is an insulin sensitizer with the ability to reduce the hepatic glucose production, improve insulin sensitivity and delay the intestinal absorption of glucose.

Indications

Management of non-insulin-dependent diabetes mellitus (NIDDM) in patients who are unresponsive to monotherapy with Metformin, Glimepiride or other hypoglycemic drugs.

MOA (mechanism of action)

The medication contains two active ingredients that work on lowering blood glucose levels via complementary mechanisms of action.

Glimepiride is an insulin secretagogue that belongs to the sulfonylurea class of drugs. Its action consists in blocking ATP-sensitive potassium channels, the closure of which results in membrane depolarization, insulin exocytosis, and calcium ion influx. This stimulates the release of insulin from pancreatic beta cells and improves insulin-response of peripheral tissues.

Metformin is an insulin sensitizer that belongs to biguanides, and it works by activating AMP-activated protein kinase (AMPK) which plays an important role in insulin signaling and fat and glucose metabolism in the whole body. This activation results in the reduction of hepatic glucose concentrations and improvement of insulin-response. Metformin also delays the absorption of glucose by the intestine and by antagonizing glucagon decreases the amount of glycated hemoglobin in the bloodstream and fasting and postprandial plasma glucose.

Dosage

Amaryl M Forte is taken orally during or after the first meal of the day and should be swallowed whole with some water.

The dosage should be prescribed for each patient individually. Maximum recommended dose is 8 mg Glimepiride/2000 mg Metformin per day.

Missed dose

If you miss a dose you should take it as soon as you remember about your missing. If it is the time for the next dose you should continue your regular dosing schedule. Do not take a double dose to make up for a forgotten one.

Overdose

An excessive dose of Amaryl M Forte can lead to lactic acidosis or hypoglycemia.

If you are experiencing symptoms of hypoglycemia, you should raise your blood sugar right away by consuming oral glucose (e.g., a small bar of sugar cubes, sweet juice, sweetened tea).

If you are experiencing some of the lactic acidosis symptoms (which include vomiting, abdominal pain, muscle cramps, severe tiredness, feeling sick, difficulty in breathing, reduced heartbeat, etc.), contact your healthcare provider immediately, as this condition may lead to a coma.

Storage

Store at room temperature between 20°C and 25°C (68°F and 77°F) away from excess moisture and heat. Throw away any unused medicine after the expiration date. Keep out of the reach of children.

Contraindications

Do not take Amaryl M Forte if:

  • You are allergic to Glimepiride or Metformin or any other ingredients in the formulation.
  • You have a severe hepatic or renal impairment.
  • You suffer from any metabolic acidosis, including diabetic ketoacidosis.
  • You are going to undergo surgery.
  • You are going to have an injection of a dye or a contrast agent for an X-ray imaging.
  • You are suffering from dehydration, severe infection, or shock.
  • You have a history of heart diseases or glucose 6-phosphate dehydrogenase (G6PD) deficiency.
  • You are pregnant or breastfeeding.
  • You are under the age of 18.

Co-administration of Amaryl M Forte with some medicines may lead to hypoglycemia, i.e.:

  • insulin
  • oral antidiabetics
  • ACE inhibitors
  • allopurinol
  • anabolic steroids and male sex hormones
  • chloramphenicol
  • coumarin derivatives
  • cyclophosphamide
  • disopyramide
  • fenfluramine
  • fenyramidol
  • fibrates
  • fluoxetine
  • guanethidine
  • ifosfamide
  • MAO-inhibitors
  • miconazole
  • para-aminosalicylic acid
  • pentoxifylline (high dose parenteral)
  • phenylbutazone
  • azapropazone
  • oxyphenbutazone
  • probenecid
  • quinolones
  • salicylates
  • sulfinpyrazone
  • sulfonamides
  • tetracyclines
  • tritoqualine
  • trofosfamide

Such drugs as acetazolamide, barbiturates, corticosteroids, diazoxide, diuretics, epinephrine (adrenaline) and other sympathomimetic agents, glucagon, laxatives (after protracted use), nicotinic acid (in high doses), oestrogens and progestogens, phenothiazines, phenytoin, rifampicin.

Thyroid hormones may weaken the blood-lowering effect of Amaryl M Forte.

Caution and close monitoring are needed in case of taking of Amaryl M Forte with any of the following:

  • phenylbutazone
  • furosemide
  • nifedipine
  • danazol
  • thiazide
  • angiotensin enzyme inhibitors (captopril, enalapril)
  • other antidiabetic drugs
  • beta-blockers
  • fluconazole
  • monoamine oxidase inhibitors (MAOIs)
  • sulphonamide
  • NSAIDs

Consult a doctor before starting the treatment with Amaryl M Forte if you’re taking cationic medications that are eliminated by renal tubular secretion.
