Description

Aerocort Rotacaps contain Beclomethasone and Levosalbutamol as the active ingredients, the combination of which is used in those patients who require simultaneous administration of both components in the treatment of asthmatic bronchoconstriction. Beclomethasone is a synthetic gluco­corticoid that helps prevent inflammation of the airways, while Levosalbutamol, a short-acting beta2-adrenergic agonist, exerts bronchodilatory activity and relaxes the smooth muscles of the airways which leads to improved breathing. The regular and long-term administration of the drug ensures relief of symptoms like chest tightness, wheezing, cough, and shortness of breath, promoting easier breathing.

Aerocort Rotacaps are not for use in acute asthma attacks, therefore, a separate ‘reliever’ inhaler for rapid relief of acute bronchospasm may be needed.

Indications

Maintenance treatment of asthma where the use of a concomitant medicine (a long-acting beta2-agonist and an inhaled corticosteroid) has been established to be appropriate.

MOA (mechanism of action)

Beclomethasone is a synthetic glucocorticoid that has a high affinity to the cytoplasmic receptors. The anti-inflammatory action of Beclomethasone includes the suppression of potent mediators of inflammation secretion (e.g., eicosanoids, leukotrienes, histamine, and cytokines) as well as the inhibition of various cells types (e.g., lymphocytes, mast cells, eosinophils, macrophages, and neutrophils), which are involved in the production of these inflammation-triggering substances. The result includes lower immune responses and reduction in the airways swelling.

BLevosalbutamol is a short-acting beta2-adrenergic agonist that activates beta-2 adrenergic receptors on the smooth muscle of the airways which leads to an increase in cyclic adenosine monophosphate (AMP). This, in its turn, prevents the release of mediators from mast cells in the airways, not allowing them to accumulate and cause asthmatic conditions, thus relaxing and opening up the airways muscles, and alleviating breathing difficulties in asthmatic patients.

Dosage

Aerocort Rotacaps are for the oral inhalation use only via a Rotahaler/Revolizer device. The capsules contain dry powder for inhalation and must not be swallowed.

Asthma management in patients 12 years of age and above:

  • 1 rotacap three or four times per day;
  • maximum dose per day should not exceed 800 mcg.

Use the medication for a prolonged period of time, even if you are asymptomatic, to get optimum benefits from the treatment. The accurate dose depends on the severity of asthma symptoms and should be prescribed by a doctor for every individual. Patients should have regular check-ups in order to make sure the dose they are receiving remains optimal and their health condition is satisfactory.

How to take the medication with the Rotahaler device:

  • Take one capsule from the packaging and insert into the Rotahaler.
  • Twist the Rotahaler to puncture the capsule.
  • After the capsule is pierced, put the mouthpiece into your mouth and grip it firmly with your lips.
  • Inhale the medicine by breathing in deeply two times through your mouth.
  • After inhaling, hold your breath for 10 seconds or as long as is comfortable.
  • Rinse your mouth with water to prevent the occurrence of mouth infection.
  • Throw away any unused capsules which were taken out of the package. Do not leave the capsules open to the air, as they may not work as well.

How to clean your Rotahaler device:

  • Separate the two halves of the Rotahaler.
  • Discard the empty rotacap shell.
  • Rinse the two halves under clean running water.
  • Shake well to remove excess water and leave it to air dry.
  • Never use your Rotahaler when it is wet.
  • Do not push any cloth or instrument into the mouthpiece as this may damage the Rotahaler.
  • Clean the Rotahaler at least once a week.

How to take the medication with the Revolizer device:

  • To open the Revolizer, hold it at the base with one hand and pull back the mouthpiece.
  • Insert a rotacap (capsule) into the rotacap chamber with the transparent end facing down.
  • Close the mouthpiece firmly. A 'click' sound will indicate a proper closing of the Revolizer.
  • Breathe out deeply through the mouth.
  • Place the mouthpiece between the teeth and seal your lips tightly around it.
  • Keep your head straight and breathe in through the mouth rapidly and deeply.
  • Remove the Revolizer from your mouth and hold your breath for about 10 seconds or for as long as it is comfortable and then breathe out normally.
  • In case some powder remains in the rotacap chamber, repeat the step 6.
  • Rinse your mouth with water after every use to avoid infection.
  • Open the mouthpiece and discard the empty rotacap.

How to clean your Revolizer device:

  • Open the mouthpiece of your Revolizer.
  • If there is an empty rotacap in the chamber, remove it.
  • Rinse the mouthpiece and rotacap chamber under clean running water.
  • Shake well to remove excess water and leave it to air dry.
  • Clean the Revolizer at least once a week.
  • Never use the Revolizer when it is wet.
Missed dose

If you miss a dose of Aerocort Rotacaps you should take it as soon as you remember. If it is the time for the next dose you should continue your regular dosing schedule. Do not exceed the doctor’s recommended dose.

Overdose

If an overdose occurs and you are feeling unwell, you should seek emergency medical attention or contact your healthcare provider immediately.

Storage

Store at room temperature between 15°C and 30°C (59°F and 86°F) and away from excess moisture and heat. The medication should not be refrigerated. Throw away any unused medicine after the expiration date. Keep out of the reach of children.

Contraindications

Do not take Aerocort Rotacaps if you are allergic to Beclomethasone, Levosalbutamol or any the components contained in the formulation.

Do not use the medicine if you have serious cardiovascular disorders, hyperthyroidism, coronary insufficiency, hypertension, and cardiac arrhythmias.

Do not take the drug if you have diabetes mellitus.

Do not use the rotacaps if you have an active or quiescent pulmonary tuberculosis.

Do not take the medication if you have low levels of potassium in your blood (hypokalemia).

Do not combine the use of Aerocort Rotacaps with such medicines as aldesleukin, aminophylline, azathioprine, cyclosporine, digoxin (used to treat heart failure), diuretic medications to treat high blood pressure (like furosemide), methyldopa, mifepristone, xanthine drugs for the treatment of asthma (such as aminophylline and theophylline).

Do not use the drug if you are pregnant or breastfeeding without consulting a doctor.

Do not give the medication to children under 12 years of age without your child’s doctor advice.
