Active Ingredient



Actos is an oral hypoglycemic used along with a proper diet and exercise program to control high blood sugar in patients with type 2 diabetes mellitus (non-insulin-dependent diabetes). It works by helping to restore the body's proper response to insulin, thereby lowering blood sugar.

The medication is composed of the active ingredient Pioglitazone, a thiazolidinedione-type and an insulin sensitizer agent, used to decrease blood glucose levels. It acts on certain adipose tissue receptors to enhance the action of insulin which leads to an improvement in insulin-dependent glucose disposal and reductions in the amount of glucose produced by the liver.

Actos is used either alone or combined with other diabetes medications (like Metformin or a sulfonylurea such as Glyburide) in the treatment of patients with insufficient glycaemic control on insulin only. Actos also has an off-label use in treating pregnant women with gestational diabetes.


Treatment of type 2 diabetes mellitus in patients who are unable to maintain their blood sugar at normal levels by adhering to a diet and exercise regimen only.

Actos is also used in an off-label way for the treatment of women with gestational diabetes mellitus when glycemic control cannot be sustained with a healthy diet and physical activities alone.

MOA (mechanism of action)

Pioglitazone is an oral anti-diabetic agent, an insulin sensitizer and a member of thiazolidinedione (also known as glitazone) medicine class. The active ingredient selectively binds to peroxisome proliferator-activated receptor gamma (PPAR-γ), also known as the glitazone receptor, disposed on adipocytes (fat cells) that affects carbohydrate and lipid metabolism, increasing the transcription of insulin-responsive genes engaged in the control of glucose production, transportation, and utilization. The activation of PPAR-γ receptors results in enhanced insulin sensitivity of liver, fat and skeletal muscle cells, a decrease in the amount of glucose produced by the liver (hepatic gluconeogenesis), and a reduction of glucose and glycated hemoglobin concentrations in the bloodstream. Pioglitazone has the ability to lower insulin resistance associated with type 2 diabetes mellitus without promoting the release of insulin from beta cells of the pancreas which excludes the risk of hyperinsulinemia.


The initiation of Actos must be based upon a blood and urine sugar level test, and thereof, should be individualized according to a patient’s condition. The medicine is meant for a once-daily use with or without food. The tablets are to be swallowed whole with a glass of water. Do not chew, crush, or break them.

Maximum daily dose of Actos is 45 mg.

During the treatment with Actos, patients should take blood and urine sugar tests periodically in order to monitor sugar levels and to assess the adequacy of response to the therapy. The medication is to be taken as an adjunct to dietary and exercise measures and its administration should not be altered or stopped even if you feel better unless advised to do so by a doctor.

Missed dose

If you miss a dose you should take it as soon as you remember about your missing. If it is the time for the next dose you should continue your regular dosing schedule. Do not take a double dose to make up for a forgotten one.

Store at room temperature under 30°C (86°F) away from excess moisture and heat. Throw away any unused medicine after the expiration date. Keep out of the reach of children.

Do not take Actos if you are allergic to its active substance Pioglitazone, any of the other medicine’s ingredients or to other thiazolidinediones (glitazones).

Do not use the drug if you are in a diabetic pre-coma or coma (a life-threatening diabetes complication that causes unconsciousness).

Do not give the medication to patients with a history of heart failure or bladder cancer.

Do not take Actos tablets if you have an altered or impaired liver function.

Do not use the medicine if you suffer from type 1 diabetes mellitus.

Do not take the tablets if you have diabetic ketoacidosis (a serious diabetes complication occurring when the body produces high levels of ketones due to lack of insulin).

Exercise caution when taking the medication if you have diabetic macular edema (DME) (a result of another diabetes complication, diabetic retinopathy, where the eye blood vessels are damaged, allowing fluid to escape).

Be careful with the medicine intake if you suffer from polycystic ovary syndrome (PCOS), as there may be a chance of getting pregnant because Actos can cause ovulation anew.

Be cautious while taking the drug, as its use may increase the risk of bone fractures, particularly in women.

Take special care while on Actos tablets, as they may cause anemia (a reduction in blood count).

Do not co-administrate the medication with rifampicin (to treat tuberculosis and other infections), gemfibrozil (used to lower cholesterol), some other anti-diabetic medicines (like chlorpropamide, glibenclamide, gliclazide, tolbutamide), and oral contraceptives.

Patients who experience visual disturbances after the medicine intake should be cautious when driving or using machines.

Do not take Actos if you are pregnant or breastfeeding without consulting a doctor.

Do not give the medicine to diabetic patients younger than 18 years of age.
